MEDICAL STATEMENT

FOOD ALLERGIES
The NHUSD FNS department does not operate allergen-free kitchens, and foods may or may not contain any of the 8 MAJOR ALLERGENS. For additional information on the the 8 MAJOR ALLERGENS click the MAJOR ALLERGENS button on the left hand side of the page.

FOOD INTOLERANCES/PREFERENCES/RELIGIOUS REQUIREMENTS
The NUHSD FNS Department understands that students may not only have particular food preferences but also specific food intolerances and religious requirements. We strive to ensure a variety of foods are offered during meal times so that as many options as possible are given to our student body. A variety of meats, vegetarian options, fruits and vegetables are offered daily during meal times.

MEAL SUBSTITUTIONS FOR MEDICAL OR DIETARY REASONS
Each special dietary request must be supported by a statement explaining the requested food substitution and must be signed by a recognized medical authority. Click the NSLP button on the left hand side for more details. The Medical Statement must include:

  • Describe the physical or mental impairment sufficiently in order for the SFA to understand how it restricts a child’s diet
  • Explain what must be done to accommodate a child’s disability
  • Identify food or foods to be omitted from a child's diet
  • Recommend food or choice of foods that must be substituted in a child's meals
  •  

EXHAUST ALL OPTIONS BEFORE COMPLETING THE MEDICAL STATEMENT

Depending on the food restrictions of the medical statement, food may not be able to be made on-site and must be prepacked and  outsourced. It is our goal that students are not ostracized during lunch time, and are able to eat foods similar to that of their peers.
